What PHI is
Information created or received by a therapist about your physical or mental health, your care, or payment for it, that identifies you. Your intake responses, the evaluation record and the letter are all examples.
How PHI is used
- Treatment. To evaluate you and write the letter.
- Payment. To bill and collect the fee.
- Operations. Scheduling, quality review, and required record-keeping.
- With your written authorization, for anything else, including sending records to a third party. You may revoke an authorization in writing.
- Where law requires or permits, including certain public health, abuse reporting, judicial and law enforcement situations, and to avert a serious threat to health or safety.
Psychotherapy notes generally require your written authorization before disclosure. Marketing disclosures and any sale of protected health information require your authorization; we do not sell protected health information.
Your landlord
We do not send anything to your housing provider. You receive the letter and you decide whether to give it to them. The letter is written without your diagnosis precisely so that handing it over does not disclose your clinical details.
Your rights
- Inspect and get a copy of your record, in electronic form if you prefer.
- Ask for a correction.
- Ask for confidential communications by a particular method or address.
- Ask us to restrict certain uses and disclosures.
- Get an accounting of certain disclosures.
- Get a paper copy of this notice.
- Be notified of a breach of unsecured protected health information.
Complaints

Effective date and changes
Effective July 30, 2026. We may change this notice and apply the change to information we already hold; the current version will always be posted here.
